[REQ] Votifier region on site page

I quite need this done to increase my traffic.
I'll be willing to pay a fair amount for it.

What do I need this plugin to do?
Have a table, similar to a poll, that says
"Vote for..."
[Slot 1] - bubble
[Slot 2] - bubble
[Bubbles being fillable]
[In-Game name]

This would replace the voting pages...

Once the player votes; it can use the plugin API from Votifier [ http://forums.bukkit.org/threads/dev-vot...-r1.22308/ ] & Votifiers economy listener [ http://dev.bukkit.org/server-mods/votifi...0-1-r1-1-1 ]to send a command to the server that the player with the [In-game name] has voted for [Slot 1/2] and will send him 600$ in-game.

Where would it be placed?
In the Index.php
Preferably underneath the header. [Two Creepers One Craft in bold]
