manual approve new registration?

I am facing problems with spammers, so I want to manual approve new registrations. Also is it possible to add a text box in the registration form, so I can understand when it is a bot or human registration (this is possible with vbulletin)

1) ACP>>Configuration>>User Registration & Profile Options. Set to Administrator Activation.

2) Also in the ACP set up a Custom Profile Field that is to be filled in during registration.
