Is there a way to maken a form, and when pushed send, it wil be published as a post in a nice way?

I would like to create an extra page in pagemanager, that will show a form,
and I would like to get the outcome of the form, so when they push "verzenden" will automaticly transfer in a post in a specific forum.
I hope this is a bit more specific?
