What's your forum URL?

This is usually a conflict between jQuery and the default JavaScript files, but also make sure that the "Show the MyCode formatting options on the posting pages" checkbox is ticked in the Edit Options menu of the User CP. It is also possible that you disabled the MyCode formatting options in the Admin CP, so go to Admin CP > Configuration > Settings > Clickable Smilies and BB Code > Clickable MyCode Editor > On.

I had this with a plugin conflict try disabling your plugins ...
