Hey, you need to create a database first from your cPanel or whatever hosting panel you have.
Next:
Server name in most cases is: localhost
Database username: It is the username with which you access PHPMyAdmin which is mostly similar to the username of your hosting panel.
Database password: It is the password with which you access PHPMyAdmin.
Database name: It is the name of the database which you created in your cPanel as instructed above, for example, example_eg

No, you don't need to upload the files again. Setup a SQL Database, and write down the values of the database in the fields you shown above.
