Would this modification completely shut out bots from registering? Will i still notice a lot of spam registration?

No, because if you get the add-on the bot will not know how to get thought the question.

No, it won't. Quality bots such as Xrumer can learn what the answer is, then remember it for the future. Also it will have no affect on human spammers. But if you use questions such as:

How much dirt is in a 4x4x4 hole?

(When obviously the answer is 'none')

Then it will minimize the chances of bots getting through.
