After adding an attachment to a reply or new thread, but before clicking Post Thread, if I middle click View My Attachments (so it opens it in another tab), and then click Post Thread, the attachment does not upload. It does not give any errors either. I am able to reproduce this over and over. I am running MyBB Version 1.6.4

What's your forum's URL? Also it would be good if you give a test account.

Give it a shot and see if it happens to you.
On a clean install of MyBB, with the default settings, I am also able to reproduce this.
