Under Manage Awards, remember the Image field is Relative to images/awards/ , therefore your image should be placed in images/awards/ and you should only type in the name of the image in the field. For example, say you had an award named welcome.gif , you would upload it to images/awards/ and type welcome.gif in the field under manage awards.

Could you create a test account with admin permissions and minimise the permissions?

You are using the wrong URL, next time look at the files you are uploading and read the readme file.
http://www.drizzymodz.com/award.php is where all of your awards will show up.
You must upload it and use the EXACT name, if you did Award.gif and put in award.gif in the plugin settings/award addition area, it wouldn't show the image.
EDIT: Don't add /images/awards to the end of your image URL, the plugin does that for you. (Hence why your image doesn't show up as well).

i did a copyed url to clipboard in ftp and put images/awards/dedicatedmembers.png
