You should have it. In plugin folder there is install.txt and it says like this:

=======
1) Copy both the extraforumperm-folder and the extraforumperm.php file into /path/to/mybb/inc/plugins/
2)
So copy folder and files from folder in your ftp, and the extraforumperm.php
Am done.. but see what happened..

Try downloading again plugin file and re-upload it.
