Somehow resolved. Image directory should not begin with .cache/, only the "Board Logo" should.
So I uploaded my logo (no longer uploaded), however, all the other "buttons" (old topic, new topic, locked topic, etc.) go to the [?] image.

The directory to my header is /cache/header/talk/logo-red.png
