I have exactly the same problem - does anyone know how this was fixed?
I'm assuming it's a CSS thing, but I haven't altered the CSS at all since it was working. I have made a couple of custom MyCode plugins, but I can't see how this could have affected it.

It was my plugin after all...
In plugin admin, I noticed the following error at the top of the page:
