Like posted before the only real way i see this possible is a minimum amount of post to see/ use the shoutbox.
With the approval. There isnt really any way of stopping proxy's and if you stop one twenty more pop up. Its a battle you simply cant win that easily, to not at all.

Guess that's a bit awkward - didn't check - there was actually a setting for "allow specific usergroups"
I've enabled email activation & disabled the shoutbox for guests, awaiting activation and banned.
