Check the header template and see if this anchor exists at the bottom:

{$atchgallery}

If not, put it there..

Also check that the atchgallery template is in global templates.

Uninstall it .... Send a fresh copy to the plugin folder in ascii format ... re-install it ... Try again

Edit:

ACP > cache manager > plugins.. if it's still not working and see if it appears in the plugins.

If the forum you set to pull image attachments from has no attachments avaliable to display, it will not show the box at all.

OK, now I really like the additions, Jeff!  Thanks for adding the global option!  

EDIT: I have to revise my initial post by saying I've turned off the gallery! I was getting mysql errors and was noticing that the Mysql queries were going well over 100 while it was active! I think there are some bugs you need to work on! Though I still think this is a neat idea!
