Step One
Don't let any attachments in posts.
AdminCP -> Configuration -> Settings -> Posting -> Maximum Attachments Per Post -> 0
This bit is the same as removing {$attachbox} in the newthread, newreply and editpost templates
Step Two
Stop Users seeing the attachment section in UserCP
AdminCP -> User & Groups -> Groups -> *All Groups* -> Attachment Options -> Can post attachments? -> Uncheck
