I'm not sure if you have CPanel or what you are using, but if you have CPanel go to your mySQL Manager. Then scroll down and you will see a box for "Assign Permissions". There you can select your mySQL username and the database you wish that user to have permissions for.
If this is not your setup, or if you have a dedicated server, post that and I will tell you how to do that.

Can you explain in more detail what happened to your forum and how this relates to the error you get in phpMyAdmin?
Most hosts disable the use of creating databases from PhpMyAdmin, I have Directadmin on my host and I can add new databases from there.
