The (1) means there's an unapproved post.

How are they unapproved and others are approved?

A post / thread is unapproved in the Inline Post Moderation & the Moderation Options (below the quick reply). You can approve it by selecting the post and selecting 'Approve Posts'.

If you want to approve all your posts at once, go to AdminCP -> Forums & Posts -> Moderation Queue.
