Logging out: If you recently changed your cookie domain or path or have not set it up please double check the settings, change if necessary and then clear your cookies.
Mixed logins: There is no logical explanation to this as the system only logs in on a valid username and password for each user. Are the users using a similar proxy server which could be caching some pages? We had a similar problem like this at high school where because of the cache server it sometimes showed us as being logged in as someone else on the same site.
