Fatal error: require_once() [function.require]: Failed opening required '/inc/class_moderation.php' (include_path='.:/usr/lib/php:/usr/local/lib/php') in /inc/functions.php on line 2218

Please reupload inc/class_moderation.php, it seams to be missing on the server.

The error says it all, ./inc/class_moderation.php doesn't exist. You need to upload it.
