Wherever you like, but headerinclude is recommended place.
You have to place it in that template which is called by every page of your board.
Also, if the icon isn't instantly noticed by IE, clear your cache, users who also cannot see it will have to clear their cache. An alternative to that is changing the file's name from, example, favicon.ico to icon.ico.
