when i checked my forum today, somebody injected "index.html" showing your site is hacked, i replaced that html immediately..
so how they injected, i have to add anything in .htaccess to protect root directory..??
You should check your FTP and hosting passwords to make sure they are secure.

I'd reset them all.
Ensure they:
- Are 8+ characters in length
- Include at least one uppercase letter
- Include at least one number
- Include at least one symbol
- Are not used anywhere else on the Internet by you
Also check your files for anything abnormal, such as a shell that could be used to re-inject you.
