If you did and that didn't fix the problem, have you modified the newthread or codebuttons templates? Please do the following(assuming you did modify them):

ACP > Templates > Modify / Delete > *Your Template Set* > Expand > codebuttons > Revert to Original

ACP > Templates > Modify / Delete > *Your Template Set* > Expand > New Thread Templates > newthread > Expand > Revert to Original
