After somebody quick replies, this image is staying until they refresh the page, despite their post being posted to the the thread.

can be java script error or plugin conflict. it requires test user account

As m says, it can also be bad HTML. Check your showthread and postbit HTML validates.

What do I need to check for?

That is the only plugin I've installed since the issue has been occuring and disabling it does not fix the error.

Does anyone know?

IIRC, it was fixed earlier. wasn't it ? above responses are still valid.
