On my website at
http://thh.no-ip.info/forum , whenever a user registers on the forum, this error is displayed:
The image verification code that you entered was incorrect. Please enter the code exactly how it appears in the image.
However, there is no CAPTCHA or any kind of verification image to fill out.
Any idea what could be causing this issue?

Nevermind I fixed it myself. I have no clue how I managed to skip over that option in the 3 times I read it carefully, but oh well.

You are using a premium theme by Audentio and I believe there's a problem with your theme, you should double check with them. Also, can you post the contents of member_register and member_register_hiddencaptcha
Go to ACP>Templates & Styles>Templates>YOUR THEME>Member Templates
The theme is technically imcompatible with the current version of MyBB (even though the download site said it was), so that probably is part of the issue.
Also, I fixed the problem, albeit by removing all captcha, which the users didn't want anyway.

My apologies for the annoyance, but it would be fantastic if this could be resolved soon as I had to disable registering new accounts because of the number of spambots.
Captcha isn't a good line of defence anyways, so no big loss for you.. To be honest, comparing to other spam preventions:
http://community.mybb.com/thread-110224.html it's really really weak. Let's say, it combats like old, rusty, naive grandpa compared to tough, shrewd kickboxer. So, since noone can find a faulty code, I'd disable it and install 2 plugins from the mentioned earlier thread. I recommend Registration Question with unique question(s) and Stop Forum Spam.
But if you really want to continue using captcha, try if it works on default style. Then again check if it works after disabling all plugins (ACP -> Configuration -> General Configuration -> Disable All Plugins? -> set to Yes -> Save) on both default and custom theme.
