Full Version: Create form on mybb page that shows results on another mybb page

Hi there, I'd like to make a form that people fill out and have the results show on another page on my forum. I don't wish to have this on a thread or a post, but on a forum page, eg:

The form:

After it's filled out, the results listed on another page:
[Image: 4TdoI.png]

Does anybody have any idea how to do this? Thanks in advance Smile
