Guys I need simple but effective bug free plugin I have found a few but they sooner or later will cause a problem any suggestion?

if you are referring to questions to users while registering for your forum then we do not have many choices
if registration security questions plugin is not working as expected then there are a couple of fixes to try (1 & 2)
there is a alternate plugin signup questions
there is a different type of plugin - checkbox validation
