Why do some people have "Double Agent" under there name on my website, I have checked the groups and it is nothing to do with them.

ACP > Users & Groups > Usertitles

Check if users are allowed to set a custom user title, if they are, then maybe a group of them decided that they want a group called Double Agent or something. Talk to them.

Okay, Post #2 thanks you i deleted it but it still shows Double Agent, I guess when someone create a new post it will not show that from now on??

No, you will have to reset it to the group default.
Run this query in phpMyAdmin

UPDATE mybb_users SET usertitle=""; 

Sorry for such a late response, I have tried that but it did not seem to work?

May be MyBB Cache's Usertitles,

Try doing a Recache at Tools & Management -> Cache...

I dont remember correctly
