Bans, disallowed IPs, disallowed emails, and disallowed usernames should all be paginated.
As part of my spam prevention, I wrote a script to grab data from StopForumSpam and add it to my database if it meets a certain threshold (eg. download listed_ip_7_all.zip and process ip addresses which have shown up at least 50 times within the past 7 days). Another script trims old or unused bans. While this means very few spammers actually get past the registration process, the ACP for banning is quite large (over 80k records between ip, username, and email).
