For the 2nd, the easiest way to reproduce is creating a forum with 'Moderate new threads' option for Registered, posting there as Registered and then approving the thread. Always results in a bug for me.
EDIT: new thread/post moderation may be actually the culprit here, but IIRC I reproduced it here in a non-moderated forum too - I may be wrong though.
Oh yeah I experienced that too here in the plugin/themes releases section IIRC.
We had a pretty big "update counters" issue not long ago - are those issues fixed now?
No, the recent issue only affected moved posts IIRC.
The last post bug can be reproduced here and is most likely caused by instant forum/group moderation:
