I would like one section on my forum to have fill out fixed form when users click on New Thread. That would mean that users can't enter anything in the new thread window except the values that are prefixed in that form.

The result after they post should be a fixed, pre formatted text that would contain the information they entered in the filled form.

Something like user created data base of certain information that would be available to everyone to browse through and use as they need.

The form should look something like this:



Is this feasible. If yes, how?

It seems too complicated for my level of knowledge on this subject. I really need this so I would be happy if someone would agree that we arrange it in that way that I pay for the job.
