I have checked all the templates, and all the codes are found in place. Also the other guide also did not help because I am not using MyAlerts with this plugin.
So what to be done now ??

I carefully check my headerinclude template but I did not find any duplication bro... Any duplication of any code.

If you have all in order then you have some broken config or any i see your code and @mmadhankumar says you have that code and many others broken on your board.

You dont have some options appearing in your board i think you have some bad vars added because thi mod is working on some sites and works all fine.
