This is something I've noticed cleaning up the moderation queue.

On a thread you can view the thread notes. These notes contain the "Last 20 Moderator Actions"

See these threads (probably only MyBB staff - sorry!)
- http://community.mybb.com/thread-159176.html - Shows who unapproved the thread.
- http://community.mybb.com/thread-165199.html - Shows who unapproved the thread
- http://community.mybb.com/thread-166044.html - DOES NOT show unapproved the POST.

Are post moderation actions not recorded in the logs?

Just approved and unapproved the mentioned post here: http://community.mybb.com/thread-166044.html Both actions show up, so maybe the previous action was just removed from logs for some reason.

Strange, perhaps it's just pruning?

Guess so. I'm quite sure it's not a bug.

But there is another inconsistency:
- if I edit someone's post message or title with quick edit or full edit it doesn't get logged
- if I edit the thread title with inline edit it's logged as "Edited post"
I don't get it at all.

And another:
- soft deleted thread titles are mentioned in information
- but hard deleted aren't
tsubject could be logged and displayed without thread link, lack of that info for hard deletion is sometimes annoying, especially when talking about posts (no title shown anywhere in the log then, quite pointless).

Also thread titles are shown in soft deletion/thread unapproval logs in 2 places (redundantly in the action message), but iIrc that has already been reported (and probably pushed).

I guess moderator actions are something that should be standardised for 2.0

@Destroy: Shall we push this thread as you found some things to fix or do you want to create seperate issues?

I think a single issue with checkboxes for all inconsistencies would be better, there are even more than I listed here IIRC, I have them noted somehwere.

I have a question regarding the Moderator Logs when approving/disapproving new posts/threads.
Is it possible that I could have the Mods required to add their notes as to why posts were not approved?
In the Mod CP, all I see is the moderator's name and that he selected a post for approval, nothing about the unapproved posts.

IMO, more information is needed for the unapproved posts and the reasons behind them. Also, the OPs' names of the said posts should be applied within the Mod notes so that we can be more aware of them making similar posts.  This is would be better used for tracking spam.
