I am looking for a plugin that displays when a user has been banned for a specific post.

It would be formated as
for ban:
"(User was banned for this post ("REASON" - BANNER))"
for warning:
"(User was warned for this post ("REASON" - BANNER))"
Warning is not 100% needed but would be nice to have, my main focus is getting a ban notice.
