Hey im trying to add this http://prntscr.com/7644vr to my admin cp so far i have this http://prntscr.com/7645qi but i want it to work like in the photo with the username box i want both awards and groups.

He want a feature in the ACP where you can upgrade or give awards in a faster way. It will basically execute a SQL query when clicking "submit" button.
