What's your board URL (link to the specific forumdisplay page which doesn't have prefixes while it should have)? Also run File Verification in ACP -> Tools & Maintenance and show the results here. List all plugins you're using.

EDIT: also take a look at: http://community.mybb.com/thread-173802.html It looks like XThreads is the culprit.
