If I do the update to 1.8.6 do I have to reinstall MyBB completely like the setup program and everything or can I just click an update button?

There is a how to upgrade page here on Mybb.com, if you're hosting your MyBB version via Installatron, you can upgrade it via Installatron.
If you manually downloaded MyBB, you also need to manually upgrade it.

You must not completely reinstall MyBB.

Please always manually upgrade it. It avoids minor issues that often result from auto-upgraders. See http://docs.mybb.com/1.8/install/upgrade/. If you still need help understanding it, we're always here to help.
