simply, yes. instant activation should be avoided.
if sending emails is not working then better to use admin activation for the registrations.

Instant activation == 80 % spam registration's
Admin activation != spam registration's
Regular mail activation >= 30% spam registration's

When our forum is new Instant Activation is good, because some people are lazy opening their email.. Or sometimes if our forum is low members.. or if we flood an advertisement in social media etc. Instant Activation ON..
if theres a member flooding/trolling our forum i banned him/moderate his post if he is not stopping i banned hir IP Address..
Just always monitor your forum and members activity..
