Hi! I don't know if there's already a plugin with this feature. We will be needing this one for our team.
The forum will be just for our team and it will only consist of 10 members.
A user will click a page and that page consist of this one.

Once a user will submit it. It will be saved to the SQL database.
SQL will consist of Submitter(the user who submit the query), Reason (the one they choose on the drop down box), Out (content in the Out) and same with In.
There will also be a page in the Admin setting where the Admin can generate the queries inserted on that date on table format. Something like.
