One of my users experienced second issue recently.
He asked me why he keeps getting private message notifications for a thread he didn't subscribed to, and he receives it even though he cancels the subscription. When I looked at thread he mentioned, (its a tournament thread and first msg needs to be updated with results) I saw one of our moderators updated that thread 16 times total and everytime moderator updated the message, thread starter's subscription preference for that thread changed to "Instant PM".
I had to ask my moderator to change his own subscription method so even if this bug happens people don't get PM.
