I have a problem with DVZ-Shoutbox..

Yeah i did all of the staps how to correctly install dvzshoutbox.
By my self designed theme http://prntscr.com/do0gav the shoutbox works but on the new theme he doesnt work?

You can't post a message in the shoutbox it will lead you to'' www.sobor.nl/?'' ??
Can someone tell me whats wrong? Cause i really cant find the problem?
