How to modify the title of submitted forms in this mode?

*form name* by *subscriber nickname*

Is suvscriver nickname the persons username?

Yes.

EXAMPLE

I'm logged with "Mark02" and I subscrive the form "Request", so the name of the submitted form is "Request of Mark02"

Use the customoutput template. You can use the same variables from the content in the title field. Just click on form name and copy and paste that to title. Same goed for username

I don't understand.
Where is that customoutput template?

In the options of the form. If not you are not running on the latest version
