The following improvements should be made to the username change system:

• Username history: Log the previous username(s) used by that user.
  
• Username approval: Require a username change to be approved by a Mod/Admin.
  
• Username change time limits: Allow only a certain number of changes in a certain time frame.
  

Yes those are needed in many communities.

1 and 3 are must haves. I'm a little eh-ish on 2, as you could ask a mod to change it for you, but it would also be useful

Yes, maybe along with option to choose how many times user can change username
