Looks like it didn't adjust to your theme too well. Someone might have to log in to your ACP and review your templates to help you or ask you to share specific templates. The plugin developer might offer support if you can contact them! This probably should have been posted in Plugin Support.

It's Focus Theme, right?
You need some code editing to make it work.
WallBB really can help you on this, he helped me before with the same issue.
But I'm putting a hold on MyAlerts right now. The plugin creator is gonna do some seriously need it upgrade on this.
