Maybe it's because you are allowing Guests to post.
Remove the permission of posting in the Guest user group and your issue is solved.

Unfortunately, Yes. Im allowing guest to create thread/reply on certain section like a "Pre-sale inquiry" where they can ask us questions before purchasing our stuffs.
Anyways, i've prevented spam by making guest only able to reply on their own thread then set every new threads/reply on moderation.
But still, if you've got any idea on how to add security question on quick/new reply it would be a lot helpful.

It's not possible, because the variables are not available in the quick reply templates.
What do you think that security question will achieve that captcha can not?
maybe switch to a different captcha method, MyBB has a few options.

I second that, use a different captcha solution and maybe a javascript based tick as well.
