Please assist I am unable to register to a forum called OpinionPals and it leads me to this error below as stated. I also attached a screenshot of it.

Make note of the section detailing the proper database configurations as shown in the
Database Configuration section.

Yes I followed everything correctly.
I’ve seen this error before usually when something in the PHP is messed up. Can you try to reupload member.php file
