turn on display of theme selector at the General Configuration settings
of forum admin panel so that someone can check above referred issue
That happens when Font Awesome stylesheet does not get loaded. Check permissions.

That is because your directory structure is incorrect as your images show.
Ok, the theme package has a folder called "upload" think of this as your forums root directory ie: yoursite.com/forum
inside upload folder is already correct structure so, copy the contents of "upload" to your forums root directory ie: yoursite.com/forum/themes/FA5Dark
ie:
yoursite.com/forum/themes
yoursite.com/forum/jscripts
