When I activate the plugin I get HTTP Error 500 

example.org is currently unable to handle this request.

I had the same problem. Check if you have installed the OUGC Awards plugins. Disable it for a while, enable MyAlerts and then reanable the awards.

Thank you. Wanted to reply for future cases if anyone else experience the same issue. Installing OUGC Awards before MyAlerts causing this error, don't know why. Just as Mixy said, deactivate them both, activate MyAlerts first and OUGC Awards afterwards. It fixes the problem!
