First of all, I want to thank mybb community for being so helpful throughout my website building process. Thank you!!

Now for the problem, when it shows last post username and profile picture, if user does not have a profile pic it appears like this:

Seems to be a theme issue, the default (no) avatar picture doesn't exist or its path is not good.

I am using BlackPlayers 1.0.3 theme, any idea where can I find the path to picture?

Can you give us the url of your forum ?
Usually the default avatar is located at images/default_avatar.png
