Please could you check if the following setting is set to yes or no?
Admin CP -> Configuration -> Settings -> General Configuration -> Disable All Plugins
If set to "yes" it needs to be "no" for plugins to function correctly.
Also want to ensure you have installed & activated the plugin at:
Admin CP -> Configuration -> Plugins
Then click Install & Activate or Activate next to MyAlerts.
