is it appearing for all forum sections ?
is it a new theme or have you recently updated MyBB version ?
probably there is a cache problem or mis-configuration.

My forum has three broad categories and it is happening in one of them including its all sub-forums under that category. I was only changing some settings (permissions) for our category wise for Moderators.
And its not in live site. Its in the localhost. As I am checking and changing various Mod permissions today.
