i try with MySubscriptions disable but still same

I'm confused. usernamehistory and MySubscriptions are two separate plugins. Did you install them both? Username History tracking is not in MyBB by default and you will need a plugin.

Looks like it's displaying the entire page twice. You can see below the error it's working, just edit the template to get rid of whatever the entire first half is.

Also might want to be careful because you're posting a hacking and illegal sharing forum when it's right in the rules not to..
